Before you begin

Have your device number ready.

If your product has not arrived, wait until you receive it. The number printed on the product is needed to connect the correct QR code to your information.

Add only information you want anyone who scans the QR code to be able to see.

Except for the device number, fields may be left blank and updated later.

New-device scans

An unactivated device may show setup instructions, demo information, or older placeholder content. That is normal.

Separate pet setup

My Doggy Tag and My Horsey Tag products use their own setup websites rather than this MAB form.

Important:

Activating and wearing the device authorizes people you allow to scan it—including professionals helping when you cannot consent—to view the information you entered. Do not include passwords, financial details, Social Security numbers, or anything you do not want visible.
